The form can be submitted in person during normal business hours (8:00am-4:30pm), faxed (515-279-3664) to or emailed.

If you elect to email the form, please email at least 2 Business days prior to your departure date. This allows our staff to distribute the information to the Police Officers on patrol. Thank you.
